Your child’s most recent psychological evaluation
Your insurance information, if available
Don’t have a current psychological evaluation? No problem. Reach out to us and we can help guide you through the next steps.
What Happens Next
Once we receive your intake form, our team will review your child’s information. If the evaluation needs to be updated to meet Medicaid criteria, we’ll let you know. A Board Certified Behavior Analyst will complete an assessment within 10 days of approval to build a treatment plan tailored to your child’s needs.
